Retinal detachment
Flashes, floaters, curtain.
- Acute PVD symptoms → dilated exam with scleral depression within 24 h; ~10–15% with acute symptomatic PVD have a retinal tear.
- Shafer sign (pigment in anterior vitreous) strongly predicts a break.
- Macula-on RRD: urgent same/next-day repair. Macula-off: within days (prognosis tied to duration).
Options: pneumatic retinopexy (single superior break), scleral buckle (young phakic, dialysis), vitrectomy (pseudophakic, complex, PVR).
